Helmsley Charitable Trust recently invested in the various projects to continue to increase the uptake of Diabetes Technologies, which are proven to improve health outcomes. Each project is unique in addressing a particular type of provider and patient population, but they all share the same goal of embedding DCES’s as part of the care model for people with diabetes, especially type 1.This session will highlight projects involving DiabetesWise Pro and CGM in Primary Care.
