Food insecurity, poverty and poor health literacy negatively impact diabetes care and outcomes. The Purple Apron program is an innovative approach to delivering diabetes care and education while tackling social determinants of health through meaningful collaboration with multi-sectoral organizations including the health clinic, local food banks, insurance plans and educational institutions. The program highlights the role of different professions in evaluating and addressing the unique needs and disparities faced by the underserved population to promote and improve population health.
