DCESs are perfectly positioned to support hospitalized people with diabetes who use or want to use continuous glucose monitors. This session will focus on strategies and tools that inpatient DCESs can use to support both patients and interdisciplinary teams. Standardization of policy and processes as well as patient safety considerations will be discussed. A novel approach to CGM education will also be shared that includes telehealth outreach after discharge.
