With the explosion of diabetes technology and related data as well as the ability to see data remotely, the diabetes care team has a huge amount of information to use to improve patient health and outcomes. This case-based presentation will demonstrate how care team members can possibly misinterpret the data when used by itself. Participants will see how patient-provided information and interactive discussion are essential to solving the data mystery to facilitate effective shared decision making and ultimately the best patient care and outcomes. Participants will learn tips for facilitating patient discussion and interacting to help solve the mystery cases presented.
