Mobile Health Applications (apps) have become powerful tools to support people with diabetes between visits where the majority of self-care occurs. This session covers how to leverage apps to help people with diabetes increase performance of self-care, achieve management goals and improve quality of life. Use of apps furthers the vision of the ADCES Identify, Configure, Collaborate (ICC) standardized framework for technology-enabled care. In this session, we will discuss how this approach may inform identification of the right app, empower the DCES to lead data-driven discussions and provide the necessary support for onboarding and collaboration with the person with diabetes to improve health outcomes. Current research will be reviewed and case studies presented to highlight the role of the DCES in the selection and adoption of apps.
