This presentation focuses on demonstrating the principles of pursuing health equity through diabetes-related community programs. These programs are (1) Pharm2Home Initiative: to deliver chronic disease and medication management outside the walls of clinics; (2) Mobile Diabetes Education Center (MOBEC): a community-based diabetes and hypertension screening program with an interdisciplinary approach. The Pharm2Home Team works closely with primary care providers and diabetologists within a system having three racially diverse, health centers (FQHCs) to adjust pharmacotherapies for managing diabetes, hypertension, hyperlipidemia, heart failure, chronic kidney disease and more, aiming to reduce therapeutic inertia. The MOBEC works in the community, urging participants to seek medical attention when screening results on blood glucose and/or blood pressure are outside the normal ranges.
