(P113) Learning From the Biggest Movers: Interdisciplinary Review of A1c Trends to Drives DSMES Improvements

The HbA1c values of patients engaged in DSMES are often integrated into an EMR registry for population health management. The Diabetes Care Team at Brooklyn Hospital has enhanced their registry to display the last two A1c values and the trend. This allows the Care Team to identify the patients with the biggest changes in A1c whose cases are reviewed at a monthly "Biggest Movers" monthly case conference. Drawing on patient statements and recent assessments, they identify patient barriers/success factors driving the A1c trends and formulate plans of action to propose to the patients. This poster will describe this interdisciplinary process and how it has contributed to patient care, process improvements and fostered collaboration across the Diabetes Care Team.
