Type 2 diabetes mellitus (T2DM) is a chronic disease that may require lifestyle changes and a structured self-care management program to provide people living with T2DM with the essential knowledge and skills and help improve their confidence to accept responsibility for their self-care management. The purpose of this quantitative, quasi-experimental quality improvement project was to determine if the implementation of the ADCES7 Diabetes Self-Care Management Program would improve the average blood glucose levels and self-care knowledge scores among adult patients living with T2DM. The statistical analysis using a paired t-test indicated a clinical and statistical improvement in the average blood glucose levels; however, there was no statistically significant difference in knowledge scores. Implementing a Self-Care Management Program is an excellent addition to diabetes management in the primary care setting, mainly when no existing structured program is available.
