Remote Patient Monitoring (RPM) is becoming more prevalent in preventative healthcare and disease management. RPM involves the use of digital technologies to capture and monitor various forms of health data such as glucose, and transmitting the data from individuals to providers to make treatment recommendations. The goal of adopting RPM platforms is to be able to monitor disease-specific data to deliver more effective care while simultaneously reducing costs through improved treatment and reduced hospitalizations. This session will highlight the value of RPM, as well as areas of confusion or challenges that arise that can hinder the benefit RPM can deliver. Understanding the data received is a key piece that DCESs can assist individuals with through RPM.
