People at risk for diabetes or living with diabetes also live with distress, trauma, loneliness, depression and thoughts of suicide. This session will take attendees through one diabetes center's journey of asking the following: How can our center identify those who need specialized services compassionately and consistently? How do we meet the dual needs of diabetes care and education and mental health care? How do we make sure seamless communication between provider types exists? How can we use processes to strengthen the care we provide? This session will share solutions found, patient stories, and explore barriers and solutions.
