Diabetes care and education continues to evolve at a rapid pace due to new medicines and changes in technology ranging from novel insulin delivery systems, insulin pumps with automated insulin delivery, advances in continuous glucose monitors, and the availability of real-time applications for mobile devices and computers. Remaining up to date with these advancements and addressing social determinants of health can be a daunting task. This presentation will review the approach utilized by an academic center's multidisciplinary advisory board. The team consists of both inpatient and outpatient healthcare professionals including nurses, dietitians, pharmacists and endocrinologists, as well as literacy experts and people with diabetes, with the goal of maximizing the care and education provided to people with diabetes.
