Diabetes is a chronic metabolic condition that disproportionately affects older adults of low socioeconomic status. The complexity of disease management and the required frequency of monitoring are particularly challenging for underserved adults aged ≥65 years due to their frailty and social determinants of health, resulting in interrupted care and ineffective diabetes management. The COACH TeAM is an evidence-based quality improvement project conducted in an FQHC that integrates community health worker home visits and same-day telemedicine appointments with primary care clinical staff to increase timely access to care and optimize diabetes self-management and education.
