Insulin regular U-500 is human regular insulin with a concentration of 500 units/mL, which is higher than the standard concentration of insulin U-100 (100 units/mL). It became commercially available in 1997 as a 20-mL vial. In 2016, a pre-filled pen and specific syringe for the 20-mL vial became available for clinical use. Insulin regular U-500 is considered for persons with diabetes and severe insulin resistance (defined as 200 units of insulin per day or 2 units of insulin per kilogram). Use of insulin regular U-500 can reduce any volume-related problems with insulin U-100 due to its higher concentration. This session will review the clinical utilization of U-500 in diabetes care, including safety concerns and processes at three different institutions.
