(P315) Inpatient Diabetes Management Improvement Projects

This poster describes how diabetes care and education specialists facilitate performance improvement projects in conjunction with the multidisciplinary team and how they will be used in the application for Inpatient Diabetes Certification from the Joint Commission. Current processes were analyzed using Lean Six Sigma. A system-wide nursing diabetes knowledge survey was completed. A system-wide audit of BGT-Insulin-Meal times was performed to identify current state. This data was used to facilitate nursing and provider education including online in-services, new nursing orientation, and CME. We plan to create a hypoglycemia heat tool as a future project.
