Helmsley Charitable Trust recently invested in the various projects to continue to increase the uptake of Diabetes Technologies, which are proven to improve health outcomes. Each project is unique in addressing a particular type of provider and patient population, but they all share the same goal of embedding DCES’s as part of the care model for people with diabetes, especially type 1. This session will highlight projects involving the financial viability of telehealth services for underserved Type 1 pediatric populations and an integrated care model for older adults with insulin treated diabetes.
