Hear from three DEAP programs as they walk you through the "doors" of their DSMES program. Learn about how they designed their service offerings and customized their programming to meet the needs of their specific target populations. Hear their success stories for their respective settings: a health system, a pharmacy, and a YMCA, in providing DSMES services for their communities.
