DCESs play an important role in diabetes care, especially in underserved populations. The DCES can help improve and maintain therapeutic inertia and reduce provider burnout by having an active role on the care team. At Federally Qualified Health Centers (FQHCs), providers have limited time and no shortage of patients to see. Utilizing various care team members such as a nurse practitioner, pharmacist, dietitian and social worker to the best of their abilities will aid in implementing an effective and holistic care plan that will help patients achieve their diabetes-related goals. This presentation will review several patient cases and discuss how each care team member played an integral part in optimizing the patient’s diabetes care plan.
