This presentation traces the evolving role and value of the DCES as discussed in recent ADCES sponsored publications in leading in integrating diabetes technology into data-driven clinical practice models so it becomes a meaningful part of diabetes care and education and not merely an add on. The presentation describes the current healthcare environment and the opening of the digital front door in chronic patient and population health management and prevention. The session will also share how the DCES leads in addressing diabetes disparities and therapeutic inertia by implementing the Identify, Configure, Collaborate Technology Framework in clinical practice.
