DCESs are experts in providing collaborative, person-centered, data-driven care within a variety of healthcare settings. In the past decade, DCESs have utilized these skills to advance national, state and local public health initiatives that prevent or delay T2D, reduce health risks for people with diabetes, and advance care for those with other cardiometabolic conditions. In this session, we will examine three public health initiatives at the national, state and local levels and explore how DCESs added value to each effort.
