Diabetes self-management education and support (DSMES) positively impacts diabetes outcomes, yet it is grossly underutilized. Diabetes care and education specialists are well-positioned to lead efforts to increase access to DSMES. Implementation science concepts can be leveraged by the DCES to facilitate DSMES delivery best practices. A diabetologist and an implementation scientist will provide a primer on implementation science for the DCES, with a focus on evaluating real-world DSMES evidence-based solutions. Real-world case studies from a regional health system will illustrate how to use implementation science concepts to increase provider referrals, offer clinical decision support in collaboration with primary care providers and endocrinologists, and engage people with diabetes in DSMES services.
