The coexistence of diabetes and cardiac disease exacerbates chronic kidney disease and morbidity. A multidisciplinary team via web based tele-mentoring for prevention and treatment of chronic kidney disease was developed. The goal was to provide support to primary physicians in Puerto Rico and locally. The project also included development of ongoing QI projects. The ECHO model was followed to share support, guidance and feedback to disseminate and implement best practices to allow them to continuously improve and expand their skills and reach.
