One critical role of the Diabetes Care and Education Specialist in the inpatient setting can be to assist in managing hyperglycemia. Even in a hospital without an Endocrinology Service, building a small Proactive Glucose Advice (PGA) team with pharmacists and registered dieticians can provide "real-time" information and recommendations to hospitalists during rounds. In collaboration with a hospital data analyst, a daily report can be built to provide information vital to assure inpatient diabetes care is based on ADA Guidelines, specifically A1c testing, initiating insulin therapy, appropriate insulin therapy based on oral intake, and discouraging the use of a sliding scale only insulin regimen.
