Diabetes care and education specialists are experts in helping patients optimize health behaviors to improve diabetes management. Traveling to meet an educator is often a barrier for accessing DSMES. By meeting patients in the community at their PCP office or through telehealth, improved attendance and collaboration can occur where the patients feel most comfortable. This poster will outline the fundamentals of developing and sustaining community based DSMES services along with utilizing this teaching methodology for the Conversation Map curriculum.
