Americans aged 65 or older have the highest incidence of diabetes-related complications. Nurse-led DSMES with follow-up calls leads to clinical improvement in diabetes management. This poster describes a proposed one-group pretest-posttest quality improvement project aims to determine if persons at least 45 years old with unmanaged diabetes who participate in nurse-led DSMES during weekly follow-up phone calls will experience improved self-management and glycemic management over three months. Anticipated results will illustrate those older patients with unmanaged diabetes benefit from nurse-led DSMES in outpatient settings. Furthermore, frequent follow-up can enhance self-management awareness. The duration of the intervention coupled with the associate benefits implies that to better assist older patients with DM management, ongoing consultation is needed outside of scheduled follow-ups in the office.
