This poster reflects the technical and clinical support being delivered by the Wyoming Center in Aging (WyCOA) teams to support DSMES, DPP, and Cardiovascular programs in our rural communities in coordination with WY ADCES Coordinating Body and it's educational membership and partners. Through an 1816 grant we are able to provide boots on the ground in our communities. The purpose is to support and provide educational opportunities that strengthen existing programs, while encouraging development and implementation in under-served or un-served high risk populations communities, improving access to care and reducing barriers to education.
